It is unfortunate that we sometimes undergo surgeries for various reasons be it elective or emergency and the desired outcome is far from favourable resulting in an immense impact on activities of daily living and social life.

This may require repeat surgery in order to correct the underlying complication.

The revision consultations that I see range from simple orthopaedic implant irritation to complete failure (malunion, non-union, sepsis, recurrence) of surgical goal resulting in severe deformities and inability to weight-bear.

It is important to understand that revision surgery is a salvage procedure and that original range of motion and appearance of the foot and ankle often needs to be sacrificed in order to have a functional and pain-free foot with a normal gait pattern.

It is also important to realise that often surgery is very complicated and higher risk to the limb and may need to be staged in 2 and sometimes 3 surgeries.

In addition to the physical examination, X-rays confirm the type and extent of the complication and other tests such as a bone scan, computed tomography (CAT) scan, or magnetic resonance imaging (MRI) may be used to evaluate your condition.

Treatment is based upon the severity of deformity and physical demands required by the patient.

Revision of foot and ankle complications

The requirements and expectations of every patient are taken into consideration, and a decision is made which would best suit your lifestyle and needs.

Conservative treatments are limited once a complication occurs and the only factor that would contraindicate a revision surgery would be if there is a general medical condition (diabetes, morbid obesity, vascular insufficiency, smoker) and this would need to be addressed prior to any revision surgery.

In the interim often custom-made braces and various rehabilitation modalities (physiotherapy, chiropractor, Biokineticist) are prescribed whilst physiological status is optimised by a relevant specialist.

This also allows time to plan the complicated surgery.

There are many types of surgery which can be performed. The decision as to which surgery is best for you is based on bone and joint factors such as the quality of the bone, the severity of the complication, the presence of any bone or joint deformity, if the rest of the foot and ankle has arthritis or is stiff, and what is the condition of the opposite foot and ankle.

Surgeries may include combinations of open , keyhole and arthroscopic techniques in addition to custom-made implants (3D printing of trabecular metal cages and replacements).
